If you are not registered with a GP practice or would like to change your registered practice, you can obtain more information about GP surgeries in your area, the services they offer and opening times by calling your Patient Advice and Liaison Service (PALS). Alternatively, visit the NHS website for more information.
We recommend that you are registered at the GP practice nearest to your home address to improve your access to medical services. Sutherland Lodge welcomes patients living within our catchment area. If you are unsure if you fall within our catchment area please contact reception who are happy to advise.
To register with Sutherland Lodge Surgery
- Contact reception to ask for the new patient registration forms.
- Complete and return with proof of address, photo ID (passport/driving licence) and, where possible, your NHS number.
- For patients new to the UK we will need your date of entry to the UK.
You will be offered a New Patient Health Check. These appointments are with the practice nurse or healthcare assistant and can be booked via reception. These appointments are a good opportunity for you to let us know a little about your history, any current medication you are taking and any concerns you may have.
