If you are not registered with a GP practice or would like to change your registered practice, you can obtain more information about GP surgeries in your area, the services they offer and opening times by calling your Patient Advice and Liaison Service (PALS).
We recommend that you are registered at the GP practice nearest to your home address to improve your access to medical services. If you would like to register at Sutherland Lodge Surgery, please ask for the new patient registration forms.
We welcome patients living within the centre catchment area. To begin the registration process please bring in proof of address and, where possible, your NHS number.
However, before you can start accessing services at the Surgery, you may be required to complete your registration process with a New Patient Health Check. These appointments are with the Practice Nurse and can be booked via Reception. The Reception staff will make it clear, when you speak with them, if you fall into this category.
In the remainder of cases a New Patient Health Check will not be necessary and your details should be on our system within two to three working days.
For patients new to the UK we need the date of your entry to the UK. We will also require to see a passport with a current work visa.
Registrations take up to 7 working days to process.
