New Patient Registration

If you wish to register you may come to the surgery and fill out a New Patient Registration Form.  We will ask you for ID and proof of address.  You will be advised to make an appointment with the Health Care Assistant for a New Patient Check.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records.
